What is Community Skilled Care and Rehab?

Swing Bed and Rehabilitative Care are terms used to describe a treatment program designed to assist patients when acute care hospitalization is no longer required, but the patient needs further care to reach a level of strength and wellness in order to safely return to their home or residential facility.

A hospital room can “swing” from acute care status to skilled care status. The actual bed/room does not change; the level of care that the patient in that room receives changes.

Community Skilled Care and Rehab at Community Hospital is a practical alternative to a prolonged acute hospitalization or short term nursing/ rehabilitation facility placement for post-acute extended care.

Who Requires Rehabilitative Care?

Patient examples include:

- Generalized weakness
- Rehabilitation following surgery
- Recovery following an accident
- Rehabilitation after a stroke or heart attack
- IV medication therapy
- Rehabilitation following an acute care inpatient hospital stay
